  • Current DUNS and SAM numbers
  • Acceptable fiscal management/stability (including ability to meet matching requirements)
  • Good standing with the Secretary of State
  • Not on any Federal or State Debarred, Suspended, Stop Payment or Excluded Parties listings

    Service Standards

  • Service providers must adhere to General Service Requirements and service-specific requirements as listed in AgeGuide's Services Standards page
