Service Description

• Meals approved by Registered Dietitians
• Meals provide 1/3 of daily nutritional needs of average older adult
• Home delivered meals include a well-being check and friendly conversation
• Eligibility: Any older adult 60 years or older (regardless of income or assets)

Eligible Applicants

• Nonprofit organizations
• Local government organizations
• Must be qualified to receive Federal/State financial assistance

Pre-Qualification Requirements

Successful applicants must comply with:
• Current DUNS and SAM numbers
• Acceptable fiscal management/stability (including ability to meet matching requirements)
• Good standing with the Secretary of State
• Not on any Federal or State Debarred, Suspended, Stop Payment or Excluded Parties listings

Service Standards

• Service providers must adhere to General Service Requirements and Title III-C Service Standards
