OCHF External Community Grants

Geographic Scope: Local community served by Osceola Community Health Foundation

Focus Areas:

• Physical health issues such as obesity, cancer, or chronic diseases
• Mental health issues such as addiction, depression, or suicide

Eligibility:

• Non-profit organizations serving the local community
• One application allowed per year per organization

Application Process:

• Applications reviewed quarterly
• Foundation director reviews applications and presents to foundation grants committee
• Foundation Grant Committee reviews and recommends to full foundation board
• Approval or Rejection communicated to applicant immediately after full board meeting

Background: OCHF provides small grants to support health-related needs in the community.

Application Process

1. Submit grant application to the foundation by one of the quarterly deadlines:
   • March 15
   • June 15
   • September 15
   • December 15
2. One application is allowed each year per organization
3. Foundation director reviews application and presents to foundation grants committee
4. Foundation Grant Committee reviews and makes recommendation to full foundation board
5. Applicant receives approval or rejection notification immediately after full board meeting
