3. Project Impact
- How many people will participate in or benefit from the project?
- How many beneficiaries will be Watertown residents?

4. Project Description
- Grant purpose and population to benefit (required)
- Statement of need and timeframe for the project (required)

5. Budget Information
- Provide a line-item budget for non-recurring capital improvement expenses (required)
- Multiple line items can be added

6. Required Supporting Documents
- Documentation of project costs (estimates/quotes from contractors/vendors)
- File types: doc, docx, pdf
- Maximum file size: 40 MB
- IRS Form 990 or 990 EZ

7. Optional Narrative
- One-page narrative supporting the request may be submitted (additional to application)

8. Consent Release Provision
- Must read and agree to consent statement
- Grants Foundation permission to photograph/film and use images, names, city, state, biographical information
- Grants unlimited rights to use images in perpetuity worldwide without additional compensation
- Images may be altered, cropped, or modified without approval rights
