Funding Source: Medicaid capitated payments through OneCareVT (Vermont's statewide Accountable Care Organization) as part of Vermont's healthcare payment reform and All Payer Model

Purpose: Provide multi-year funding for organizations serving the NVRH service area to create healthy and thriving communities with positive social, economic, or environmental impact

Transportation

Education

Arts and culture

Prevention and wellness initiatives

Social determinants of health

NEK Prosper Outcome Areas (Must align with at least one):

Eligibility Requirements:

Projects must contribute to healthy and thriving communities by addressing social determinants and prevention opportunities

Must accelerate the work of NEK Prosper

Must be tied to at least one of the NEK Prosper outcome areas
