Geographic Scope: NVRH service area (Caledonia and Southern Essex counties, Vermont)

Funding Source: Medicaid capitated payments through OneCareVT (Vermont's statewide Accountable Care Organization) as part of Vermont's healthcare payment reform and All Payer Model

Purpose: Provide multi-year funding for organizations serving the NVRH service area to create healthy and thriving communities with positive social, economic, or environmental impact

• Healthy food production and access
• Transportation
• Education
• Arts and culture
• Prevention and wellness initiatives
• Social determinants of health

NEK Prosper Outcome Areas (Must align with at least one):

• Well-nourished
• Well-housed
• Mentally healthy
• Physically healthy
• Financially secure

Eligibility Requirements:

• Projects must contribute to healthy and thriving communities by addressing social determinants and prevention opportunities
• Must accelerate the work of NEK Prosper
• Must be tied to at least one of the NEK Prosper outcome areas
