Program Description School-Based Health Centers (SBHCs) are a safety net health care delivery model located in or near a school facility that provides students with high quality comprehensive medical care, including preventive and behavioral health services. These services are provided by licensed and qualified health professionals in accordance with federal, state, and local law. Additional services provided by a SBHC can include but are not limited to: chronic medical condition management, including diabetes and asthma care, acute care for illness and injury, oral health care, vision care, nutritional counseling, reproductive health care, substance abuse care and counseling, referral to a specialist, medical home, or hospital for care, services that address social determinates of health and emerging services such as mobile health and telehealth. Category 1: The New and Emerging Grant is a onetime opportunity to provide funding to organizations that are planning to open and operate one or more new SBHC sites. The grant provides three years of funding to plan, organize, and open a new SBHC site. By the end of year two it is expected that a new SBHC site will be ready to open and students may begin receiving services in year three.Eligibility
