Matt Salmon Nurse Education Training Scholarship

Award Amount: $2,500 annually

Purpose: To help cover education costs associated with earning an LPN, RN, or BSN degree for one Hacienda HealthCare employee annually. Established to recognize Matt Salmon's commitment as a long-term advocate (25+ years) for Hacienda's clients, patients, and their families.

Geographic Scope

• Arizona (Hacienda HealthCare location)

Eligible Nursing Degrees

• LPN (Licensed Practical Nurse)
• RN (Registered Nurse)
• BSN (Bachelor of Science in Nursing)

Eligibility Requirements

• Must be employed by Hacienda HealthCare (full-time or part-time) for at least 1 year and in good standing
• Must have at least one year left of school/training
• Currently pursuing further education in nursing with LPN, RN, or BSN degree
• Applicant must attend an accredited school of nursing
• Maintain a C average or higher upon submitting application
• Applicants must describe financial need on the application
• Selection based on merit and financial need

Scholarship Use

May be used for:
• Tuition
• Books
• Lab fees
• Supplies
• Day care

Recipient Responsibilities

• Maintain a C average throughout schooling
• Repay scholarship if dropping out or grades fall below C average
• Report at end of year on how scholarship was used and helped further medical training

Selection Process

• Scholarship committee consists of three management team members
• Award given to one eligible employee who has performed beyond expectations in caring for Hacienda patients/clients
• Selection considers financial need for assistance

Application Timeline

• Application Period Opens: November 1 (annually)
• Application Deadline: December 1 (annually)
• Recipient Announcement: At the annual Hacienda Holiday party (date TBD), announced by CEO

Note: Scholarship is not renewable, but recipients may reapply each year
