3. Recipient Announcement: At annual Hacienda Holiday party by CEO (date TBD)

7. Are you currently attending school? (Yes/No)
8. If not currently attending, have you been accepted? (Yes/No)
9. What school are you attending or planning to attend?
10. Describe your financial need in managing school expenses
11. Are there any special personal or family circumstances affecting your need for financial assistance?
12. How are you currently financing school?
13. How are you planning to finance the remainder of school expenses?
14. Explain in 150 words or more why you chose nursing or the medical field
15. What are your career goals after receiving your nursing degree?
