Matt Salmon Nurse Education Training Scholarship

A $2,500 annual scholarship established to support nursing education for Hacienda HealthCare employees pursuing advanced nursing degrees.

Purpose: Recognizes Matt Salmon, advocate extraordinaire, whose commitment of time and energy has had a significant positive impact on Hacienda's clients, patients, their families, employees, and supporters. The scholarship helps cover education costs for nursing degrees.

Geographic Scope: Arizona (Phoenix area)

Focus Areas:

• Nursing education
• LPN (Licensed Practical Nurse) degrees
• RN (Registered Nurse) degrees
• BSN (Bachelor of Science in Nursing) degrees

Eligibility Requirements:

• Must be employed by Hacienda HealthCare (full-time or part-time) for at least 1 year and in good standing
• Must have at least one year left of school/training
• Must be pursuing further education in nursing with LPN, RN, or BSN degrees
• Must attend an accredited school of nursing
• Grades must be a C average or higher upon submitting application
• Must describe financial need on the application
• Selection based on merit and financial need

Award Details:

• Amount: $2,500
• Renewable: Scholarship is not renewable, but recipient is allowed to reapply each year
• Use: Tuition, books, lab fees, supplies, day care, etc.
• Selection: One scholarship awarded annually to eligible employee who has performed beyond expectations in caring for Hacienda patients/clients and has demonstrated financial need

Selection Committee: Three management team members choose the scholarship recipient

Recipient Responsibilities:

• Maintain a C average
• Repay scholarship if dropping out of school or grades drop below C average
• Report at end of the year on how the scholarship was used and helped further medical training
