Live Well at Home Grant

Administrator: Minnesota Department of Human Services (DHS)

Geographic Scope: Minnesota statewide; administered locally through CMCOA

Program Purpose: Works with partners statewide to expand the capacity of long-term services and supports to help people age 65 and older stay in their homes and communities of choice.

Grant Focus Areas:

Address challenges of aging and forecasted pressures on Minnesota's long-term support system by 2030

Improve chronic disease management in Minnesota's communities

Expand long-term services and supports capacity by linking formal and informal services

Support caregivers and promote independence through market-based solutions

Eligibility: Communities and providers in Minnesota

Technical Assistance: CMCOA ElderCare Development Partnership (EDP) is available to provide technical assistance to applicants. Consultation will focus on project need and design, outcomes, sustainability, budget, as well as review of draft proposals. Applicants are strongly encouraged to contact a Community Development team member for assistance as early as possible.
