Assistance programs for prescription eyeglasses offered by various Kentucky Lions Clubs. Not directly affiliated with KLEF; the Foundation distributes applications only. Guidelines vary by club.

General Eligibility Requirements:

• Individual must be financially in need
• Must have no vision insurance or limited insurance coverage
• Application must be completed and submitted to local Lions Club
• Must provide verification of need for glasses
• Must provide documentation of monthly income support (income statement, food stamp letter, disability letter, paycheck stub, etc.)

• KLEF does not provide assistance for prescription eyeglasses
• KLEF does not have authority or ability to approve or expedite applications
• Guidelines vary among different Lions Clubs
• Application processing times vary: average waiting time 2 weeks to 6 months

How to Apply for Eyeglass Assistance in Jefferson County

1. Identify Your Zip Code: Determine which Lions Club serves your area (40118, 40201-40299)
2. Download Application: Click on your zip code to download the appropriate application
3. Complete Application: Fill out the downloaded application form
4. Gather Documentation: Provide with application:
   • Verification of need for glasses
   • Documentation of monthly income (income statement, food stamp letter, disability letter, paycheck stub, etc.)
   • Do not send original documents
5. Submit by Mail: Return the printed application by mail to the address listed on the form
6. No Printer Access: If you cannot print, call 502-583-0564 to request application mailed to you
7. Wait for Response: Lions Club volunteers will review and notify you via phone or mail (2 weeks to 6 months typical wait time)
