The Knapp Community Care Foundation Grant supports organizations in the Mid-Valley region that enhance health and wellness. Since 2013, the foundation focuses on projects addressing the Social Determinants of Health, including economic stability and healthcare access. Eligible organizations must be recognized 501(c)(3) entities and demonstrate innovative programs that positively impact the community. The foundation encourages partnerships and evidence-based initiatives to leverage additional funding and improve health outcomes for vulnerable populations.

Overview

Knapp Community Care Foundation was created in 2013, and since its inception has created a mission of serving the Mid-Valley community through supporting organizations that promote the improvement of health in Mid-Valley families. We are proud to partner with organizations serving the deep South Texas Mid-Valley community. In order to receive funding, an organization must be a qualified public entity or an Internal Revenue Service recognized 501(c)(3), 509(a), or Section 170(c)(1) with current Form 990s and in good standing with the IRS and relevant state and local authorities. We approach all grant-making through the lens of the Social Determinants of Health. Requests must meet at least one of the five Social Determinants of Health: Economic StabilityEducation Access & QualitySocial & Community ContextHealthcare Access & QualityNeighborhood & Built Environment We encourage projects that are evidence or research-based, projects that have the capacity to leverage additional funding, and collaborative partnerships.

Eligibility

In order to receive funding, an organization must:Be a qualified public entity or an IRS recognized 501 (c) (3) in good standing with the IRS, and must provide programs and/or services directed at improving wellness, fitness, nutrition services, preventative health and health education access within the service delivery area.Offer unique, innovative, or enhancement of programs and/or services that demonstrate a positive effect on the individuals in our community that need it most.Cities in the Catchment Area of the FoundationWe fund organizations serving the deep South Texas Mid-Valley community. This includes the communities of:San CarlosLa BlancaDonnaHargillLa FeriaLa SaraLyfordMercedesProgresoRaymondvilleSanta RosaSebastianWeslacoincluding the Delta area: Edcouch, Elsa, La Villa, Monte Alto, and other rural communities within the Mid-Valley region.Organizations are not required to have a physical location in the Mid-Valley as long as they provide services in these communities.An organization does not have to be physically located within the Mid-Valley area, but the programs and/or services must be provided to Mid-Valley residents and in the Mid-Valley area. We encourage programs that include: Evidence/researched based modelsLeverageCollaborationsPrograms/services must address one of the five social determinants of health.
