Joining Hearts Grant Application

Geographic Scope: Community-based (Atlanta, GA area)

Mission Alignment: Grant requests must align with Joining Hearts' mission statement and address current community needs.

Grant Period

• Submission Window: March 1 to October 31 (annually)
• Review Timeline: All submissions reviewed in November
• Notification: Recipients notified in December

Eligibility Requirements

• Organization must have an IRS Determination Letter (Form 501(c)(3) or equivalent)
• Organization must have a defined mission statement
• Organization must be able to submit quarterly reports on fund utilization

Reporting Requirements

If selected, organizations are required to submit quarterly reports to the Joining Hearts Board of Directors outlining how allocated funds were utilized.

   • Maximum file size: 32 MB total
   • Maximum number of files: 5
4. Submission Process:
   • Complete all required fields in the online form
   • Attach proposal and IRS Determination Letter
   • Submit through the grant request form on joininghearts.org
5. Review and Notification:
   • Board of Directors reviews all submissions in November
   • Recipients notified in December
