Grant Application Process

Submission Requirements:

1. Complete the Donation/Grant Request Form (mandatory)
- All fields marked as required must be completed
- Submission deadline: 15th of month preceding board meeting

2. Required Form Sections:
- Organization name, address, phone, email, tax ID (EIN or SS#)
- Confirmation of 501(c)(3) status
- Contact person and date of submission
- Assigned Sportsmen's Club board member liaison

3. Project Description:
- Brief description of organization and mission alignment
- Specific detail of how funds will be applied
- For multiple items/services: upload Word, Excel, PDF, or JPEG with itemized cost breakdown
- Maximum file size: 25 MB

4. Program Details:
- Total amount of funds requested (excluding admin/transportation costs)
- Number of youth involved
- Cost per child calculation
- Geographic area youth are from
- How request specifically relates to Sportsmen's Club mission
- Is this a one-time event or ongoing? (annual, bi-annual, etc.)
- Timeline: latest date approval needed (MM/DD/YYYY format)
- Is this time-sensitive?

5. Funding Information:
- Percent of total cost the Sportsmen's Club would contribute
- Whether you've requested additional funding from other nonprofits and amounts
- Willingness to accept partial donation (if yes, specify what % match acceptable)
- Previous donations from Sportsmen's Club (year and amount)

6. Partnership & Visibility Agreements:
- Confirm board members may visit/participate at no cost
- Grant permission for photos to be taken for publication
- Willing to provide photos from event for banquet/website?
- Availability to present request at board meeting (yes/no)

7. Optional Materials:
- Cover letter detailing charitable impact (optional)
- Maximum file size: 25 MB, accepts PDF or DOC format
