How to Apply
Grant Recommendation Process
Fund advisors use the Grant Recommendation Form to request distributions from their Donor Advised Fund.
Required Information:
1. Advisor Name: Full name of the fund advisor
2. Fund Name: Name of the Donor Advised Fund
3. Recipient Organization Details:
- Name and address
- Phone number
4. Grant Amount: Suggested distribution amount
5. Signature and Date: Advisor signature acknowledging:
- Final judgment rests with Grants Committee
- Distributions must align with Foundation purposes
- No legal obligation or expected goods/services in return
Submission Format: Physical form or electronic submission
