Healthy Community Fund It is the goal of the fund to help support organizations that are advancing community health through collaborative and evidence-based practices targeting the priority health needs in Washington County. Purpose The Healthy Community Fund’s highest funding priority is given to programs or projects that address the hospital’s priority health needs, social drivers of health and/or target underserved populations. Funding is available for non-profit organizations in good standing that represent a range of demographics within Washington County. Qualifications IRS tax-exempt organization(s) classified as a 501(c) that provides services in the Froedtert West Bend Hospital service area (Washington County). Projects that: Contribute to improving the overall health status of the community, including social determinants of health:Education (e.g., Literacy programs, higher education programs, health education programs)Employment (e.g., Soft skills training, apprenticeship programs, internship programs)Health (e.g., Access to quality care, healthy food, screening, behavior health)Safety (e.g., Transportation, housing, green space access, violence education & prevention)Establish new programs or support existing initiatives that improve health and wellnessSupport communities in Washington County, specifically marginalized communitiesFoster collaboration between organizationsIncorporates population health and Froedtert West Bend Hospital Community Health Improvement Plan priorities Please keep in mind that Froedtert West Bend Hospital’s Healthy Community Fund does not provide funding for the following: Requests that benefit an individualPolitical campaigns and/or elected officialsFundraising initiatives
