Froedtert West Bend Hospital Healthy Community Fund Grant

The Froedtert West Bend Hospital Healthy Community Fund aims to enhance community health in Washington County by supporting programs that address priority health needs and social determinants. This fund prioritizes non-profit organizations focused on underserved populations, fostering collaborative efforts that improve health outcomes. Qualifying projects include health education, job training, and community safety initiatives. The fund excludes individual benefit requests and political campaigns, ensuring resources are directed toward community-wide health improvements.

It is the goal of the fund to help support organizations that are advancing community health through collaborative and evidence-based practices targeting the priority health needs in Washington County.
Purpose
The Healthy Community Fund’s highest funding priority is given to programs or projects that address the hospital’s priority health needs, social drivers of health and/or target underserved populations. Funding is available for non-profit organizations in good standing that represent a range of demographics within Washington County.
Qualifications
IRS tax-exempt organization(s) classified as a 501(c) that provides services in the Froedtert West Bend Hospital service area (Washington County).
Projects that:
Contribute to improving the overall health status of the community, including social determinants of health:Education (e.g., Literacy programs, higher education programs, health education programs)Employment (e.g., Soft skills training, apprenticeship programs, internship programs)Health (e.g., Access to quality care, healthy food, screening, behavior health)Safety (e.g., Transportation, housing, green space access, violence education & prevention)Establish new programs or support existing initiatives that improve health and wellnessSupport communities in Washington County, specifically marginalized communitiesFoster collaboration between organizationsIncorporates population health and Froedtert West Bend Hospital Community Health Improvement Plan priorities
