Foundation for Health Equity (f4HE): Community Health Grant

The Foundation for Health Equity (F4HE) is dedicated to addressing systemic health inequities in Northwest Philadelphia. Through strategic partnerships, they empower organizations to tackle issues like food insecurity and promote healthy aging. Grants are available for 501(c)(3) nonprofits in specific zip codes, supporting both immediate needs and systemic change. F4HE emphasizes advocacy, community engagement, and sustainable health solutions to foster vibrant neighborhoods where everyone can achieve optimal health.

Grants
As a grantmaking organization, Foundation for Health Equity provides direct support to organizations fulfilling immediate, individual needs as well as addressing historical, systemic inequities.
Grants can be awarded to nonprofits that fit F4HE’s grant-receiving criteria under one of our primary focus areas: Food Insecurity, Healthy Aging, Women’s and Children’s Health. These grants are awarded to those non-profits that are exemplary of BOTH direct service and systems change efforts.

Eligibility

Grants are restricted to organizations identified as tax-exempt under section 501(c)(3) of the Internal Revenue Code and serving Northwest Philadelphia in the following zip codes:
19150 – Wadsworth19144 – Germantown19138 – East Germantown19128 – Roxborough19119 – Mt. Airy19118 – Chestnut HillOne LOI per organization may be submitted in a single calendar year. Applicants will be notified directly should there be any exception.

Ineligibility

Eligibility does not guarantee funding
Organizations must be identified as tax-exempt under section 501(c)(3) of the Internal Revenue Code and cannot identify as a private foundation. A photocopy of the organization’s most recent IRS determination letter is required for consideration of funding.
Foundation for Health Equity operates under a mission-driven strategic plan which guides our grantmaking. Therefore, as a general practice it is outside of our funding scope to provide grants for the following:
