Overview

Our Mission Foundation for Health Equity supports communities in NW Philadelphia by building on the assets of organizations that work to address systemic health inequities in order to build healthy, vibrant neighborhoods. Our Vision We envision a community where everyone has a fair and just opportunity to achieve optimal health. Our History Initially called Chestnut Hill Health Care Foundation, we were established in 2005 with the proceeds of the sale of Chestnut Hill Hospital to carry out charitable activities intended to improve the health of those living in the hospital’s primary service area. Transitioning names but not our mission, the Foundation became Green Tree Community Health Foundation in 2008. For 13 years the Foundation worked diligently to improve the health of those affected by hunger, illness, and lack of health insurance. In 2021, at the conclusion of a full-scale strategic planning process, the Foundation decided to change our name in order to better reflect the mission: Foundation for Health Equity (F4HE). Having listened to our community partners during the strategic planning process, we added advocacy and community engagement to our work. This allows us to better support and invest in the nonprofit partners who directly serve community members. Grants As a grantmaking organization, Foundation for Health Equity provides direct support to organizations fulfilling immediate, individual needs as well as addressing historical, systemic inequities. For example, we fund programs that feed hungry families as well as work to end food deserts. We also fund programs that help older adults age in place as well as protect their rights. Our goal is to improve systems that have historically made it difficult for people to gain and maintain health. Grants can be awarded to nonprofits that fit F4HE’s grant-receiving criteria under one of our primary focus areas: Food Insecurity, Healthy Aging, Women’s and Children’s Health. These grants are awarded to those non-profits that are exemplary of BOTH direct service and systems change efforts.

Airy19118 – Chestnut HillOne LOI per organization may be submitted in a single calendar year. Applicants will be notified directly should there be any exception.

Ineligibility

Eligibility does not guarantee funding Organizations must be identified as tax-exempt under section 501(c)(3) of the Internal Revenue Code and cannot identify as a private foundation. A photocopy of the organization’s most recent IRS determination letter is required for consideration of funding. Foundation for Health Equity operates under a mission-driven strategic plan which guides our grantmaking.
