The Wyoming County Community Health Foundation (WCCHF) awards grants to programs and organizations that improve the health and wellness of people in Wyoming County, PA and surrounding communities.

Current Limitations:

• Funding Cap: Temporarily limited to grants under $5,000.00
• Not Funded: Capital improvements are not considered at this time

• Vision: To be recognized as a collaborative and important funding source for programs that provide enhanced health and understanding of health and wellness in communities

Evaluation Criteria (Scoring Rubric):

1. Meets a significant Wellness and/or Health Care Need in the Community (weight: 10)
2. Number of People Served (weight: 5)
3. Significant Impact on People's Lives (weight: 4)
4. Systematic Improvement vs. Band-Aid or Token Effort (weight: 4)
5. Partners with Other Community Resources (weight: 6)
6. Addresses Multiple Community Needs to Maximize Return for Investment (weight: 9)
7. Helps Sustain Community Hospital (weight: 7)
8. Enhances WCCHF Reputation and Provides Results Supporting Planned Giving (weight: 6)
9. Professional Execution of the Initiative (weight: 6)
10. Cost Benefit (Affordability, Total cost, Cost Per Person Served) (weight: 8)
11. Total Cost/Grant Amount Requested (weight: 5)
12. Application Reflects Use of Evidence-Based Best Practices (weight: 6)

Timeline:

• Letter of Intent Deadline: February 1st (annually)
• Grant Application Deadline: March 31st (annually)
• Review Date: October (Program Committee evaluation)
• Funding Decision: Approximately 1-2 weeks from review meeting date
• Fund Availability: Within 4 months of funding decision

Application Process

Step 1: Submit Letter of Intent

• Deadline: February 1st of each year
• Submission: Email to WYoffice@wyoming-county-health.org (Executive Director, Shealynn Shave)
• Required Content:
  • Description of the project or activity
  • Explanation of how the project/activity fits WCCHF mission and goals
  • Estimate of the amount to be requested

Step 2: Submit Grant Application (if Letter of Intent is accepted)

• Deadline: March 31st of each year
• Important Note: Acceptance of the Letter of Intent does not guarantee funding approval

• Program Committee evaluates applications in October
• Funding decision made approximately 1-2 weeks from review meeting date (unless additional information needed)
• Funds made available within 4 months of funding decision
