Geographic Scope: United States with emphasis on:

  • National programs promoting scalable and sustainable systemic change

  • Local programs in: California, Florida, Georgia, Indiana, Missouri, Nevada, New York, Ohio, Texas, and Virginia
  • Focus Areas:

  • Improve clinical health outcomes for individuals with diet-related conditions

  • Increase access to nutritious foods for populations experiencing food/nutrition insecurity

  • Target populations experiencing high social risk
  • Grant Program Goals:

  • Improving clinical outcomes for individuals with diet-related conditions experiencing high social risk

  • Increasing access to nutritious foods for populations experiencing high social risk and food/nutrition insecurity
  • Eligibility:

  • Qualified nonprofit organizations across the U.S.

  • Must be exempt from federal income tax under section 501(c)(3)

  • Must be a public charity described in section 509(a)(1) or (a)(3)

  • Must not be a section 509(a)(3) type III nonfunctionally integrated organization
  • Grant Terms:

  • Generally 1-3 years in duration

  • Grant ranges are open; funding levels determined based on need and plan presented

  • Organizations cannot receive overlapping grants from the foundation
  • Status: Grant applications will be accepted again in 2027
