Food as Medicine Grant

Geographic Scope: United States with emphasis on:

• National programs promoting scalable and sustainable systemic change
• Local programs in: California, Florida, Georgia, Indiana, Missouri, Nevada, New York, Ohio, Texas, and Virginia

Focus Areas:

• Improve clinical health outcomes for individuals with diet-related conditions
• Increase access to nutritious foods for populations experiencing food/nutrition insecurity
• Target populations experiencing high social risk

Grant Program Goals:

• Improving clinical outcomes for individuals with diet-related conditions experiencing high social risk
• Increasing access to nutritious foods for populations experiencing high social risk and food/nutrition insecurity

Eligibility:

• Qualified nonprofit organizations across the U.S.
• Must be exempt from federal income tax under section 501(c)(3)
• Must be a public charity described in section 509(a)(1) or (a)(3)
• Must not be a section 509(a)(3) type III nonfunctionally integrated organization

Grant Terms:

• Generally 1-3 years in duration
• Grant ranges are open; funding levels determined based on need and plan presented
• Organizations cannot receive overlapping grants from the foundation

Application Process

1. Review the overview for Food as Medicine focus area
2. Download the Request for Proposal (RFP) which details required information
3. Access and complete the grant application using the Apply link (applications available in 2027)
4. Note: Applications are not currently being accepted; check back for 2027 deadline

Required Materials

• Complete application through foundation portal
• Follow all specifications in the RFP document
• Application should detail how program addresses the focus area priorities

• Applications must be downloaded and saved locally before submission as they cannot be accessed or revised after submission
