    Background: Brylan's Feat Foundation is a non-profit organization that maintains exemption under IRS section 501(c)3. The foundation is the first and only nonprofit in the world specific to pediatric lymphedema.

    How to Apply

    Application Process

    Step 1: Submit Required Documents
    Upload in Questionnaire or email admin@brylansfeat.org:

  • Lymphedema diagnosis from doctor

  • 3 to 5 photos of patient (at least one photo including face)

  • Copy of insurance card (US recipients)

    • Step 2: Complete Questionnaire

  • Access questionnaire through application portal

    • Step 3: Complete Required Forms

  • Waiver of Liability

  • Photo/Info Release Form

  • PT Disclosure Form

    • Returning Recipients:

  • Recipients who have not been in contact with BFF for more than 1 year must fill out an update form

    • Processing Time:

  • Standard: 5-7 business days after application completion

  • June & July: 10-14 business days
