  • Improve long-term health outcomes for individuals and the community
  • Impact the underserved
  • Demonstrate a strong community need
  • Promote partnerships and collaborations within the community
  • Demonstrate potential for continued operation beyond Board of Visitors support
  • If previously funded, must have submitted Final Grant Usage Report by February 1st

    Funding Considerations

    Will consider funding for:
  • Seed money for specific projects
  • General operating support
  • Capital campaigns to raise a specified sum of money within a defined time-period to bring needed healthcare services to the community

Financial Documentation

Organizations selected to submit full proposals must submit an IRS Form 990 for the past 1-2 years.

    Restrictions

  • One request per organization per calendar year
  • When applying for Category C, an organization may also apply for Category D

Application Process

1. Grant Request Stage: Submit a grant request via the online grants portal by the deadline
2. Selection: Applications are reviewed and selected organizations are asked to submit full proposals
3. Proposal Review: Submitted proposals are reviewed and site visits are scheduled with selected organizations
4. Award Notification: Grants are awarded at The Board of Visitors Grants Award Celebration in February
