Application Process

Two Application Options:

#### Option 1: Financial Aid Short Form
For returning families with minimal changes to financial situation

Required Information:
1. Parent full name
2. Parent email address
3. Parent cell number
4. Income change verification (over 10% in past two years?)
5. Occupations and gross salary/income of each working household member
6. Number of new students attending this year
7. Number of children paid tuition/childcare fees for in previous school year
8. Current payments to other schools and programs
9. Annual charitable giving amount
10. Other sources of income, investments, parsonage/housing allowance, or nontaxable funds/benefits
11. Additional information for committee consideration
12. Copy of most recent tax return (required upload)
13. Signature and date certification accepting 10% tuition increase and confirming unchanged financial situation

#### Option 2: Financial Aid Full Application
For new families or those seeking same/greater aid than previous year

Step-by-step 8-step form:
1. Parent's Information: Father's and mother's contact information and addresses
2. Children's Information: List of children attending Bais Yaakov and other children
3. Father's Income: Gross wages, dividends, interest, business income, and other income sources
4. Mother's Income: Same income categories as father
5. Children's Income: Taxable income, dividends, and assets
6. Household Assets: Business assets, real estate, investments
7. Household Expenses: Housing, auto expenses, unusual expenses, charitable giving
8. Aid Information: Full tuition amount, requested payment amount, financial aid requested
