The Department has three primary branches – Public Health Services, Health Systems, and Integrated Health. All work collaboratively toward improving health by strengthening New Jersey’s health system. Our Mission Protect the public’s health, promote healthy communities, and continue to improve the quality of health care in New Jersey. Federally Qualified Health Center Expansion Program NOFA To provide reimbursement to Federally Qualified Health Centers (FQHCs) supported under Section 330 of the Public Health Service Act and Centers designated as FQHC look-a-likes (FQHC LAL) to provide primary care, dental, and mental health services to the uninsured and underinsured.

Federally Qualified Health Centers (FQHCs) supported under Section 330 of the Public Health Service Act or federal designation as a FQHC look-a-like (FQHC LAL). The FQHC/FQHC LAL must be licensed by the Department of Health with the ability to deliver primary medical, dental and/or mental health care to uninsured/underinsured residents. Applicants must have a valid NJ Tax Clearance Certificate and a valid NJ Charities Registration Letter of Compliance submitted with their application.
