Application Process

Submission Method: Online application with uploaded documents

Key Deadlines:

Application submission deadline: Thursday, June 5, 2025 at 5:00 PM CST (no exceptions)

Grant Committee notification: On or before August 1, 2025

Grant report due: Monday, June 1, 2026

1. Overall purpose of organization - specific description of how you serve participants, including how organization addresses Emergency Food Assistance and Homelessness
2. Mission statement and how it is carried out in the community (include full mission statement)

#### Program Specifics and Participants (750 characters max per question)
1. Describe the program/project to be funded as it relates to Emergency Food Assistance and/or Homelessness
2. Explain specifically how grant funds will be used

#### Population Served (750 characters max per question)
1. What percentage of population served are DuPage residents?
2. What percentage of this specific program/project participants are DuPage residents?
3. Which specific community or communities will benefit?
4. How many individuals will be served?

#### Program Type and Sustainability (750 characters max per question)
1. Is this a new, continuing, or one-time program/project? (Select one)
2. Plans for fully funding and sustaining the program/project to meet Emergency Food Assistance and/or Homelessness needs

Program budget (showing how grant funds will be used)

IRS 501(c)(3) determination letter

Board members list (employer, position; if retired, former profession)

Demographics and Impact data (families served, individuals served, intake criteria, length of client service, geographic area served, referral information)

3. File Upload Requirements:

Maximum file size: 32 MB per file

Format: All documents submitted online

Confirmation and Follow-up

• Confirmation email sent after application receipt
• Check spam folder if confirmation not received within 10 minutes
• Grant Committee will notify via email of acceptance or denial by August 1, 2025
• Selected organizations must submit grant report by June 1, 2026
