Overview

2025 Arts & Community Health Request for Proposals The Horizon Foundation for New Jersey (The Foundation) will open the 2025 Arts & Community Health RFP to continue promoting the powerful role the arts can play in improving health. The arts are an effective tool for conveying health messaging and encouraging healthy behaviors within a community. Our goal is to support new and innovative arts programs to help empower our members and the communities we serve to achieve their best health. The Foundation supports programs that provide opportunities to engage the communities we serve in prevention, education, and awareness. Through a competitive application process, we invite arts organizations to submit a grant application that will advance one or more of the following areas of focus: Directly encourages healthy behaviors—physical and/or mental healthPromotes greater dialogue on health—specifically the priority areas of obesity, behavioral health, or social determinants of healthEducates, connects to, or raises awareness of available health servicesMakes health information more clear, memorable, and relevant to the community Examples: A community theater program that powerfully conveys the risks of opioid use disorderAn arts-making partnership with a community health office to raise awareness of programming on diabetes and nutrition The intersection of arts and health is a growing field, and the Foundation is eager to accept thoughtfully constructed proposals in this area.

Eligibility

Criteria for qualified applicants:Arts organizations that promote or leverage fine, visual, performing, and/or literary arts through programming that engages individuals, audience members or the community-at-large.Arts organizations with previous health-focused programming experience is welcome, but not required.Arts organizations may partner with a health or other organization to design and/or deliver programming; applicants must describe in detail the collaborative partnership in their proposal.The Horizon Foundation for New Jersey only considers requests:Submitted by organizations located in New Jersey that have tax-exempt classifications under Sections 501(c)(3) of the U.S. Internal Revenue Code.To fund state or regional offices of organizations with multiple chapters or multiple affiliates operating in New Jersey.To fund local chapters or affiliates of organizations with state or regional offices by invitation only.New Jersey universities and accredited colleges may submit one grant application per calendar year through their institution’s foundation.Organizations should meet the following criteria:Have an established mission that promotes health or the arts in New Jersey communities.The program to be supported must align with the Foundation's funding priorities and be able to report on our measurement indicators. This information can be found on the Grantee Information page.Be able to identify specific and measurable short- and long-term objectives.Be located in and serve New Jersey communities.Have demonstrated experience with the activities described in the funding proposal.Does not duplicate the work of other organizations already supported by the Foundation within the same geographic area.Have a current and satisfactory financial audit conducted by a Certified Public Accountant. This is required for all organizations, including those whose incomes are under $500,000.Agree to the Foundation’s financial and narrative reporting requirements for monitoring purposes.Have a longstanding reputation for credibility and integrity.

Ineligibility

The Horizon Foundation for New Jersey will not fund:Arts programs that address general wellness.Arts programs that provide therapeutic services.Proposals received from a non-Arts organization.Proposals that fall outside of our New Jersey service area.Multi-year grant requests.The Horizon Foundation for New Jersey will not fund:Capital campaignsRenovations or new buildingsEquipment onlyEndowmentsHospitals or hospital foundationsIndividualsPolitical causes, candidates, organizations and campaignsOne-day eventsFundraising eventsGalas, dinners or golf outingsWalks, runs, races and similar events
