This transition created new challenges and costs for organizations and families who struggled during the pandemic. The Foundation authorized this special action fund to help friends and neighbors emerge from the COVID pandemic.

- If applicable, describe how previous SCCR Foundation grants were utilized and their impact on the community
6. School Status: Confirm whether organization is a school or part of school district
7. Required Supporting Documents (confirm ability to provide):
- Cover Letter
- Profit & Loss Statement
- Balance Sheet
- Proof of IRS Tax Exemption
- Copy of Most Recent Federal 990 Form
