COVID-19 Action Fund: Non-Profit Emergency Grant

Application Requirements

1. Application Form with the following information:
- Organization Name
- Name and Title of Contact Person
- Address
- Phone Number
- Email
- Website Address
- Amount of request with itemized breakdown of how funds will be used

2. Organization Information:
- Length of organization existence
- Description of organization's purpose and importance
- How organization impacts local community

3. COVID-19 Impact Statement:
- Describe how organization has been negatively impacted by COVID-19 pandemic, OR
- Describe how organization is directly addressing challenges created by coronavirus outbreak and shelter-in-place orders

4. Geographic and Utilization Information:
- Geographic area where grant funds will be utilized
- Detailed description of how emergency grant funds will be used if approved
- If applicable, describe how previous SCCR Foundation grants were utilized and their impact

5. School Status: Confirm whether organization is a school or part of school district

6. Required Supporting Documents (confirm ability to provide):
- Cover Letter
- Profit & Loss Statement
- Balance Sheet
- Proof of IRS Tax Exemption
- Copy of Most Recent Federal 990 Form

7. Submission: Include name and title of person submitting application
