Program Description: Community Partnership Grants provide sustained, flexible support to core Lakewood organizations serving priority populations through multi-year general operating grants. These grants embody HLF's commitment to trust-based philanthropy, offering flexible funding that helps organizations thrive and directly advance the health and well-being of Lakewood residents.

Focus Areas

Grants support work addressing:

  • Basic needs and stability: Housing stability and affordability, food security and access, safety, and access to essential services

  • Mental health services: Mental health counseling, prevention programs, crisis support, and trauma-informed care

  • Education: Education access, early childhood, and school readiness

  • Community connection: Social connection, community engagement, and health-promoting community programs and spaces
  • Priority Populations

    • Low-income families and individuals
    • Children and youth
    • Racial and ethnic minorities, including refugees and immigrants
    • Older adults
    • Persons with disabilities
    • LGBTQIA+ community members
    • Uninsured and underinsured residents

    Grant Terms

    • Amount: $7,500 - $50,000 per year
    • Duration: One or two years of funding
    • Type: General operating support or programmatic support depending on organizational size, capacity, and context

    Eligibility Requirements

    Organizations are eligible to apply if they:

  • Do work that directly impacts Lakewood residents (including organizations based elsewhere that provide significant programming or services in Lakewood)

  • Have been operating for at least three years

  • Are a 501(c)(3) nonprofit organization or have a fiscal sponsor
  • Review Criteria

    Rather than scoring or ranking, HLF reviews applications holistically, considering:

  • Impact in Lakewood: How work directly benefits Lakewood residents, particularly those facing barriers to health

  • Mission alignment: Connection between your mission and HLF's commitment to health equity

  • Organizational capacity: Current capacity and aspirations for growth

  • Community connection: Evidence of community trust, cultural responsiveness, and meaningful engagement with people served
  • What This Grant Does NOT Support

    • Capital campaigns or building projects
    • Fundraising events or event sponsorships
    • Lobbying activities beyond those permitted for 501(c)(3) organizations
    • Individual scholarships
    • Religious or evangelical purposes
    • Organizations that discriminate based on age, race, national origin, ethnicity, gender, disability, sexual orientation, political affiliation, or religious beliefs

    Application Process

    Cycle One: February 17 - March 11, 2026 (CLOSED)

  • Application open

  • March 23 - April 24, 2026: Application review and interviews

  • May 2026: Final funding slate approved by HLF Board
  • Cycle Two: August 17 - September 16, 2026

  • Application open

  • September 28 - October 30, 2026: Application review and interviews

  • November 2026: Final funding slate approved by HLF Board
  • Application Steps

    1. Submit online grant application - HLF's streamlined online form helps them understand your organization, your work in Lakewood, and how funding would support you
    - View PDF of grant application for reference

    2. Request assistance if needed - Ask for help with the grant application process

    3. HLF staff and committee review - Staff and Grants and Learning Committee identify strong fits for the grant cycle

    4. Interviews (if selected) - May be invited to a conversation with committee members to discuss your work and ask questions about partnering with HLF
    - Interview invitation (or lack thereof) should not be interpreted as indication of final funding decision

    5. Funding decision - HLF Board of Directors makes final decisions based on Committee recommendations

    6. Grant agreement - Review and submit signed grant agreement in online portal in timely manner

    7. Reporting - Submit final grant report and/or discuss learning with HLF staff
    - May be asked to share information on spending related to awarded grant

    Post-Award Expectations

    • Act as responsible steward of funding and ambassador to the community
    • Provide feedback on application, implementation, and reporting phases to improve future opportunities
    • May be asked to share ideas with other funded groups and identify areas of synergy and shared impact
