Community Outreach Grant Program
Funder: Jewish Community Foundation of Greater Prescott
Mission: In keeping with the 5000 year-old Jewish tradition of helping one's neighbor and the act of Tikkun Olam (repairing the world), the Foundation utilizes unrestricted funds from generous donors to support new, innovative, and start-up charitable projects.

Focus Areas
The Foundation supports grant proposals in the following areas:- Organizations must be IRS-recognized direct 501(c)(3) charitable organizations
- Organization and program must provide benefits and services to the quad-city area of Yavapai County
- Priority given to: - New or substantially improved programs - Programs that promote community partnerships - Programs with demonstrated community need and clear benefits - Programs with demonstrated sources of continued funding beyond grant period
- Lower priority for: Organizations receiving significant funding from state or federal government

Required Information for General Organizations:
1. Name of applicant organization

5. Brief bio of executive director
6. Names of trustees of organization
7. Brief summary of the organization's mission
8. Concise description of the project
9. Total cost of the project and the amount requested from the Foundation
10. Who in the community will benefit from the project
11. Start and completion dates of the project
12. How will the outcomes of the project be evaluated
13. IRS letter of determination of tax-exempt status
14. Total funding budget and prior year resources
15. Organization's most current financial statements and most recently filed tax return
