Application Instructions

Account Setup:

1. Create an account (new users) or login with existing credentials
2. Recommended: Use your Agency Name as your User Name
3. Watch for confirmation email to set password (check spam folder)
4. Keep username and password for future agency reporting and applications

Application Completion:

1. Complete all sections in the online application portal
2. Do not leave any blanks; enter "N/A" if not applicable
3. Click "SAVE DRAFT" frequently to save progress
4. Click "SUBMIT" when completed (cannot access after submission, but copy will be emailed)

• Program Budget
• IRS Nonprofit Determination Letter (NEW APPLICANTS ONLY - if applicant has fiscal sponsor/agency, provide proof of agreement and fiscal agent's sponsor's IRS Nonprofit Determination Letter)
• Board of Director's List
• Statement of opinion from most recent Independent Auditor's Report (cover sheet) OR statement explaining why not available (NEW APPLICANTS ONLY)
• For grants requesting over $10,000:
  • Most Current Balance Sheet
  • Organizational Budget

• Late applications will NOT be accepted
• Incomplete applications will not be accepted
