    Focus Areas: Grants focus on one or more health priorities outlined in the 2024-2025 Community Health Needs Assessment and FY 2026-2028 Strategic Implementation Plan. Specific priority areas include:

  • SDOH (Social Determinants of Health) & Chronic Disease
