Overview

Community Health Investments Grant Program The Owensboro Health Community Health Investments Grant Program provides financial assistance to nonprofit organizations, governmental or public entities who are working to address identified priority health issues and those social determinants of health that impact the health and health outcomes of our community. Organizations requesting grant funding should review one or more local community health needs assessments and/or community health improvement plans to substantiate their request. All activities must serve individuals within the defined Owensboro Health service area. Owensboro Health seeks and supports partnerships to impact the significant health issues facing our communities. As tobacco use is a factor related to multiple health issues in our region and state, it is our decision to make opportunities for investments to those organizations, like Owensboro Health, which have committed to establishing and implementing 100 percent tobacco-free policies. It is required that all applicants to our Community Health Investments Grant Programs (both large and mini-grants) show evidence of a 100 percent tobacco-free policy. Large Grants Owensboro Health offers grants up to $50,000 to eligible organizations serving 11 specific counties in Kentucky and Indiana, focusing on programs that address health needs and promote health equity. Applications are accepted annually. Projects must demonstrate a clear connection to improving health outcomes and be tied to an identified health need in the community. Programs for which funds are requested must focus on providing activities or services that address an identified health need or that address social determinants of health. The connection between health needs and the activity or service must be clearly stated on the application.

Eligibility

Recipients must be established entities, such as 501(c)3s or government agencies, and are required to submit quarterly progress reports. Grants to Section 501(c)(4) organizations may be considered if the proceeds are used exclusively for section 501(c)(3) permissible purposes, are not used for lobbying, and are not used for political campaign activities. A 501(c)(4) recipient must report back to Owensboro Health on its use of the grant proceeds. Further, grants to 501(c)(4) organizations will not be permitted to cover the recipient’s fundraising costs or to provide general support. A 501(c)4 recipient, like all grant recipients, must provide a quarterly report to Owensboro Health on its use of grant proceeds.Owensboro Health will only consider applications by schools and school-affiliated groups and activities that have been authorized by the school’s central office. Please include proof of authorization from your school’s central office with your application.

Ineligibility

Partisan political groups or activities, endowments, memorials, individuals, field trips or tours are not eligible for funding.
