Overview

Community Health Cohort Pilot

Award Amount: Estimated minimum of $5,000 per organization (Board discretion for additional amounts) for participation in collaborative and strategic work.

Purpose: Invite organizations to work together to solve persistent community problems through collaborative and strategic work, leading to community-level action supported by organizational work and sector leadership.

Strategic Priority Areas:

  • Healthy starts for children: Support for Healthy Children's Fund implementation and community/provider-readiness

  • Youth Mental Health: Advancing the Connecting Care recommendations

  • Aging Well Whatcom: Advancing the Aging Well Blueprint

  • Civic Belonging and Engagement: Building grassroots capacity and movement-building

  • Opioid Response and Supports: Advancing the All-Hands Whatcom recommendations
  • Cohort Activities:

  • Review community recommendations for action

  • Consider innovative approaches

  • Establish/support community learning agendas

  • Discuss opportunities for collaboration on shared goals

  • Move from plans on paper to catalyzing community and organizational actions
  • Expectations for Participating Organizations:

  • Active participation in meetings (schedule to be established at first convening)

  • Willingness to work iteratively and learn openly with colleagues

  • Passion and excitement for collaboration toward transformative work

  • Willingness to participate in a pilot and help establish guidelines and process in partnership with the Foundation

  • Shared fundraising/grant-writing assistance provided as needed
