- Letter of Intent: Addressed to Program Director describing reasons for pursuing residency and career goals
- Curriculum Vitae (CV): Must include: - Education and Training - Professional Experience - Experiential Education - Professional Memberships - Leadership Experience - Honors and Awards
- Leadership Statement: Response to "What is your philosophy on leadership?" (500 word limit)
- Reference Information: Names with degree/credentials, titles, employers, phone numbers, and email addresses for three (3) references
- Letters of Recommendation: Three (3) letters via online recommendation form
- Transcript: Unofficial transcript with application; official transcript from pharmacy school via email to Program Director
Interview Process
- Phase: January through February - Virtual interviews with program and site - Medication therapy management (MTM) case exercise - Applications reviewed by Application Review Committee
Post-Selection
- Candidates notified of status on or around March 1
- Successful candidates complete onboarding processes starting in March
- Residency year begins on or around June 15
