Comeback Assistance Program

Purpose: Assist and support stroke and heart disease survivors and caregivers on their personal comeback journeys by providing financial assistance for recovery-related costs.

Does NOT Fund:

• Existing medical and/or personal bills
• Home health care
• Travel and/or lodging
• Medications
• Insurance
• Home improvements
• Down payments for a home
• Mortgages/rent

Eligibility

• Stroke or heart disease survivors
• Caregivers of stroke or heart disease survivors
• Program currently only accepting applications for stroke or heart disease related inquiries

Background

Established to help survivors who may lack the support system and opportunities that founder Tedy Bruschi had during his stroke recovery in 2005. Recognizes that the financial burden outside of what health insurance covers can be overwhelming for survivors and their families.

   • Confirmation of survivor status (required)
   • Inquiry for family member (required)
2. Automated Response: An automated email will be sent to the email address provided containing a link to the Comeback Assistance Program Questionnaire
3. Complete the Questionnaire: Fill out the detailed questionnaire form
4. Coordinator Follow-up: Lauren Pino, the Comeback Assistance Program Coordinator, will connect with you directly for further information
