Geographic Scope
- Applications must be authorized and submitted by an existing non-profit organization
- Organizations do not need to be Episcopal Churches, but if not, a copy of IRS tax exemption certificate must be enclosed
- Organization must be located within the geographical boundaries specified (east of Worcester County, MA)
Eligibility Requirements
- The organization has moved all funding requests to an online application portal
- Organizations can create a profile, save drafts, and submit applications online
- Emailed applications are no longer accepted, even for organizations that have received previous funding
- Organizations requiring assistance with the portal should email: tchs@diomass.org
Application Process
- Grantees must submit a Post-Award Grant Evaluation Form at 10 months into the program/project
- Final expense report must accompany evaluation (3 pages maximum)
- Reports should address: accomplishments, steps/actions taken, progress measures, unexpected results, setbacks, other funding sources, and sustainability plans
- Reports should be mailed to: The Church Home Society, 138 Tremont Street, Boston, MA 02111 or emailed to: tchs@diomass.org

Online Application Process
1. Create an Organizational Profile
- Visit the online application portal using the link provided on their website
- Create an organization profile
2. Complete Grant Application
- Once profile is created, create a new application
- Applications can be saved as drafts before final submission
- Submit application online through the portal
3. Required Documentation
- For non-Episcopal Church organizations: Copy of IRS tax exemption certificate
- All other required application materials as specified in the online portal
4. Post-Award Reporting (10 months after grant award)
- Submit Post-Award Grant Evaluation Form
- Include Final Expense Report showing how grant was used
- Responses limited to 3 pages maximum (plus expense report)
- Address the following in your responses:
- Major accomplishments
- Steps or actions used to meet objectives and goals
- Measures used to determine progress
- Unexpected results or key learnings
- Any setbacks encountered and how they were addressed
- Other funding sources for the project
- Plans for sustainability beyond the grant period
