BIPOC-led community organizations

Organizations working in underserved communities

Organizations with proven impact and potential to shape community change

Award Structure:

Unrestricted operational funding of $25,000 per organization

One-year awards

Opportunity for additional funding in future years

Access to leadership development and capacity building support

Additional Support Provided:

Facilitated leadership cohort

One-on-one leadership coaching

Interactive group coaching sessions

Implementation of Healthy Together digital referral platform (powered by UniteUs)

Tailored learning curriculum including leadership development training and interactive workshops

Data collection support for social determinants of health

Addresses resource gaps in nonprofit sector

Reduces burden of multiple applications and reporting requirements

Promotes collaboration across philanthropic sector

Amplifies voices of traditionally under-resourced community leaders
