Award Details:

Two hospitals honored annually within the LifeLink of Georgia service area

Announcement: January at Georgia Hospital Association's Patient Safety and Quality Summit

Nomination Criteria:
Hospitals should demonstrate support of organ and tissue donation through specific examples including but not limited to:

Dramatic increase in donors or referrals due to hospital champion, donation committee, or team efforts

Significant process changes that improved donation outcomes or family experience

Community donation events that increased donation awareness

Memorials or events honoring patient donors

Collaboration with local media on donation and transplant stories

Eligibility:

Open to any hospital within LifeLink of Georgia service area

Anyone can submit a nomination (not self-nominations only)

Status: Nominations are closed for 2025 cycle

About the Award:
Named after John and Barbara Ware. John received a lifesaving kidney transplant and he and Barbara became passionate advocates for organ donation. After Barbara's stroke, she became an organ donor, continuing their shared mission of saving lives through donation.

1. Eligibility: Any hospital in LifeLink of Georgia service area

2. Who Can Nominate: Anyone (donor families, transplant recipients, healthcare professionals, community members, etc.)

3. Submission Method: Online nomination form

4. Information Required in Nomination:

- Specific examples of initiatives, events, or activities demonstrating commitment to organ and tissue donation
- Examples should address one or more nomination criteria:
- Donor/referral increases with specific efforts documented
- Process improvements with measurable outcomes

- Media collaboration examples

5. Selection Process:
- Selection committee reviews all nominations received by deadline
