- Project title
- Description/Public Health Relevance Statement

- Letter of support requirement (Yes/No)
- Primary and additional funding sources
- IRB approval requirement (Yes/No)
- Existing IRB number (if applicable)
- Non-disclosure agreement requirement (Yes/No)
- Detailed description of required resources (data, tissues, organs, or other resources)

3. Acknowledgement Requirement:
- Must agree to acknowledge LifeLink Foundation support and donor generosity in any resulting publications, presentations, abstracts, grant applications, or posters

4. Submission:

- Response within five business days with preliminary feedback and next steps
