Overview

The Altman Foundation's Health program invests in NYC programs that: (1) expand access to health care for underserved, uninsured or vulnerable populations; (2) expand and strengthen quality out-of-hospital/community-based care for the most vulnerable (priority to systems-change, sustainable models, reducing avoidable hospital use); and (3) develop the workforce for primary, community-based and at-home care and care management. Eligibility: U.S. 501(c)(3) organizations (not private foundations) operating within New York State, focused almost without exception on the five boroughs of New York City; no grants to individuals, and no funding for bricks-and-mortar/capital, galas/fundraising events, budget deficits, or public universities. Observed 2026 grants in this area ranged roughly $35,000-$200,000 (FIA: Inference from the 2026 grants list; no fixed grant sizes are published, so amount fields are left null).

Entry is by Letter of Inquiry through the Foundation's online grant portal; if invited, applicants submit a full proposal (Direct Service, Capacity Building, or Systems Change). Note: the Foundation currently has limited funds for new, non-foundation-initiated grantmaking.
