ALM Scholarship Renewal Application

Required Information:
1. Student Information:
- Full name (First and Last)
- College Student ID number
- Email address
- Phone number
- High school attended

2. Parent/Employment Information:
- Name of parent employed by ALM Family of Companies
- Name of specific company where parent is employed
- Summer employment (if applicable): Will you be employed during summer break? If yes, list company name and start date

3. Academic Information:
- Type of degree: Two Year or Four Year
- Upcoming year in school: Freshman, Sophomore, Junior, or Senior
- College or University attending (name and full address including city, state, zip)
- Your major/field of study
- Credit completion: Did you complete minimum 15 credits last semester? (Yes/No)
- Credit maintenance: Will you maintain minimum 15 credits this semester? (Yes/No)
- GPA verification: Did you maintain GPA of 2.5 or higher last semester? (Yes/No)

4. Essays (for Summer Renewal Application only):
- Educational and career goals: Discuss your educational and career goals and how you plan to achieve them (limited space)
- Personal goal: Discuss a personal goal and how you plan to achieve it (limited space)

5. Required Attachments:
- Transcript with grades from previous semester
- Official class schedule for upcoming semester
- Accepted file types: JPG, GIF, BMP, DOC, DOCX, PDF
- Maximum file size: 256 MB

6. Attestation:
- Must certify that all information is true and correct
- Must authorize La Crosse Community Foundation to obtain information from academic records and transcripts to verify eligibility

7. Submission:
- Complete all required fields
- Submit via online form
- Confirmation email will be sent to provided email address upon successful submission
