NTRAC is committed to providing AED units to qualifying community organizations that demonstrate need, readiness, and commitment to training and device maintenance.- Organization Types: Nonprofit, Government, Educational, Religious, or Other

Eligibility Requirements
- Organization must allow NTRAC to conduct CPR/AED training for staff, volunteers, or community members
- Organization commits to maintaining the AED in good working order (battery and pad replacement)
- AED must remain accessible to the community as agreed upon
- All applications must be complete; incomplete applications may not be considered

1. Complete Application Form - Fill out all sections of the AED Program application
2. Provide Applicant Information:
- Organization/Group Name
- Primary Contact Person
- Title/Role
- Phone Number
- Email Address
- Mailing Address
3. Provide Location & Community Impact Information:
- Address where AED will be placed
- Description of community or population served
- Approximate number of people served annually
4. Demonstrate Financial Need:
- Answer whether organization currently has funding to purchase AED
- If no, explain any financial hardship or lack of resources
5. Confirm Commitment to Training:
- Agree to allow NTRAC to conduct CPR/AED training
- Provide estimated number of participants to be trained
- Note: Training scheduling will be coordinated by NTRAC after approval
6. Confirm Maintenance Commitment:
- Agree to maintain AED in good working order
- Identify person(s) responsible for ongoing maintenance
7. Submit Supporting Documentation (if available):
- Proof of nonprofit status (if applicable)
- Documentation of financial need (if applicable)
- Letter of support from leadership/board (optional)
8. Certification:
- Sign certifying all information is true and accurate
- Confirm organization commits to NTRAC-provided training
- Confirm organization commits to proper placement and maintenance
- Confirm AED will remain accessible to community
- Include your name, title, today's date
9. Return Application - Submit completed application to NTRAC Regional Advisory Council for review
