Access to Cologuard Testing (ACT) Program — Clinic Partnership (Colon Cancer Coalition)

The ACT program is an in-kind award — the Colon Cancer Coalition supplies Cologuard colorectal-cancer screening kits (50 or 100 over 12 months) to partner clinics, plus optional 2D barcode scanners and a demo kit for patient education, rather than cash. Applicants must describe how patients with a positive Cologuard result are navigated through to a colonoscopy, including coverage and gaps in that care.

Clinics apply via the ACT Program Clinic Application form. The application asks for a project overview and expected outcomes, community need, patient follow-up pathway for positive results, number of kits requested (50 or 100 over 12 months, or another amount) and delivery option (direct mail, on-site, or hybrid). Stated requirements: up to three months from application to first order; orders placed through the EpicCare Link Portal; staff training through Exact Sciences on the third Thursday of each month; a 2D barcode scanner is required for on-site or hybrid distribution (up to 2 can be provided). Partners submit quarterly updates at 3, 6, 9 and 12 months, with the 12-month report covering follow-up colonoscopy completion, cancers detected and demographics.
