2025 Agency Funding Application

Application Process

1. Account Setup:
- LOGIN if you already have a username and password
- CREATE AN ACCOUNT if you are a new user (confirmation email will be sent)
- RESET YOUR PASSWORD if needed
- Keep username and password for future use (required for agency reporting and future applications)

2. Download Required Templates:
- Download Agency Summary Budget (Excel Sheet)
- Download Program Summary Budget (Excel Sheet)
- Contact JaMelle Hill (jhill@unitedwaycg.com) or Tammie Collins (tcollins@unitedwaycg.com) if you have trouble accessing documents

3. Complete Application:
- Fill out all sections completely
- Do not leave any blanks; enter "N/A" if not applicable
- Use "Save a Draft" feature to save your progress

4. Upload Required Documents:
- Program Summary Budget (Excel Sheet, new this year - complete for each program)
- Full Program Budget(s)
- Agency Budget Summary (Excel Sheet, new this year)
- Full Agency Budget
- Most recent 990
- Most Recent Audit or Financial Review
- 501(c)(3) Status
- Charity Registration with Georgia Secretary of State
- Board of Directors' list
- Agency Logo

5. Submit Application:
- Submit by 5 pm on June 30th
- Be sure to log out once you have submitted your application
