YPC Grant Application Process

Final Report Submission (Required for Re-application):
1. Submit YPC Grant Final Report form
2. Include the following information:
- Organization name
- Grant amount and grant number
- Program name
- County (Clark or Floyd)
- Project description
- Project leader's name, phone, email, website, social media addresses
- Name and contact information of person completing form

3. Program/Project Information Required:
- Project start and end dates
- Number of youth and adults involved
- Target population vs. actual reach
- Final project format
- Adherence to original plans and explanation of any changes
- Problems encountered and how they were resolved
- Community need addressed
- Impact assessment on identified issues
- Research on complementary work by other organizations
- Adult involvement (leaders, staff, volunteers)
- Youth involvement and capacity
- Number of beneficiaries
- Youth empowerment and leadership development
- Expectations conveyed to participants
- Social competencies developed (teamwork, decision-making, friendship building)
- Results versus expectations
- Most important project outcomes
- Feedback on granting process

4. Required Attachments:
- Copy of final budget
- Receipts, brochures, public announcements, photos, newspaper clippings
- Any materials supporting project or program

5. Submission Options:
- Complete online form

6. Authorization:
- Form must be signed by authorized person with signing authority for organization
