Overview

Youth Homelessness Demonstration Program (YHDP)

Status: Award already received (August 2019)

Geographic Scope: Suburban Cook County, Illinois (all 130 suburbs outside Chicago)

Focus Areas:

  • Prevent and end homelessness for youth ages 25 and younger

  • Development and implementation of coordinated community plan

  • Youth system navigation and coordination

  • Interim and long-term housing supports

  • Youth-specific access points to homeless housing and services

  • Identification of youth experiencing homelessness through outreach and school partnerships

    • Key Accomplishments:

  • Housed 111 young people in 100 days during pilot challenge

  • Assessed vulnerability of 95% of youth in system

  • Connected with 30 school districts

  • Identified 304 unhoused youth under 18 in suburban Cook County schools during one school year

    • Target Population:

  • Unaccompanied youth under 25

  • Young adults with children

  • Youth with history of foster care, incarceration, or both

  • Youth experiencing homelessness due to family homelessness, parent/child conflict, running away, parental struggles

    • Program Components:

  • Youth system navigators

  • System mapping and collaboration

  • Authentic youth engagement through Youth Action Board

  • Collaboration with education, juvenile justice, child welfare systems
