Wild Blue Dogs Cancer Treatment Grant

Purpose: Provides financial assistance for cancer treatment for dogs whose owners lack the financial resources to afford treatment.

Geographic Scope

• Eligible regions: California and immediately adjoining communities only

Eligibility Criteria

• Dog must be diagnosed with cancer
• Owner must demonstrate financial need and inability to afford cancer treatment
• Owner must provide:
  • Copy of 2 recent bank statements
  • Copy of previous year's tax return statement
  • (Copies reviewed and destroyed after evaluation)
• Dogs trained to perform work or tasks for people with disabilities receive priority
• Owner must agree to treatment course mutually decided by Wild Blue Dogs and attending oncologist/veterinarian
• Owner must be willing to provide volunteer participation with Wild Blue Dogs or CLEAR events
• Owner must sign release permitting use of patient's story and images for promotional purposes (no private/financial information disclosed)

Priority Considerations

• Priority given to patients with best prognosis with treatment versus without treatment
• Priority given to dogs trained for service/disability assistance work

What is Covered

• Chemotherapy
• Radiation therapy
• Laser treatment
• Surgery and related surgical costs
• Medications for symptom treatment, side effects, and pain management
• Emergency euthanasia costs (for dogs already receiving grant, sudden emergency situations only)

What is NOT Covered

• Unexpected side effects and new conditions
• Other emergency services (non-euthanasia)

Treatment Facilities

• Cancer treatments and surgeries must take place in facility selected by Wild Blue Dogs
• Only specified facilities with explicit written authorization from Wild Blue Dogs will receive payment
• Wild Blue Dogs reserves right to select alternative veterinarians/facilities if initial location is impractical

Selection Process

• Minimum of two Wild Blue Dogs Board/Advisory Members review applications
• All board decisions are final
• Names of decision-making board members not released outside Board of Directors
• Applicants assigned patient identification number for unbiased selection
• Both owner and veterinarian notified via email of committee decision

Application Process

Step 1: Owner Application

• Download and complete the Owner Application Form from wildbluedogs.org
• Provide:
  • Copy of 2 recent bank statements
  • Copy of previous year's tax return statement
  • Documentation of financial need and inability to afford treatment

Step 2: Veterinarian Assessment

• Veterinarian must complete the Veterinary Notes/Assessment Form
• Veterinarian submits form directly to Wild Blue Dogs via email
• Include all pertinent test results (lab work, x-rays, sonograms, etc.)

Step 3: Submission

• Owner emails or mails application form and required financial documents to:

• Veterinarian emails assessment form and test results to:

Step 4: Review and Matching

• Wild Blue Dogs matches veterinarian records with owner's application form
• Applicant and dog assigned patient identification number for unbiased selection

Step 5: Veterinary Oncology Review

• Wild Blue Dogs team of veterinary oncologists reviews records
• Team makes treatment recommendations

Step 6: Selection Committee Review

• Form given to selection committee for final review and decision
• Minimum of two Wild Blue Dogs Board/Advisory Members make final decision

Step 7: Notification

• Both owner and veterinarian notified of committee decision via email
