Wayne Memorial Health Foundation Community Grant Program
Organization: Wayne Memorial Health Foundation (WMHF)
Mission: To support community organizations whose endeavors are focused on injury and disease prevention and treatment and the promotion of good health for residents in the Wayne Memorial Health System service area.
Vision: A progressive, culturally and economically healthy community supported by an active, locally-controlled healthcare system dedicated to enabling the highest possible quality of life for all of its residents.
- Primary service area: Wayne and Pike Counties, Pennsylvania
- Also serves Carbondale and Forest City, PA areas
Geographic Scope
- Applicant organizations must be 501(c)(3) non-profit corporations
- Must provide health care services, health education, and human service programs that improve health and well-being of community residents
- Service/project/program must not be an unnecessary duplication of services already provided by another organization in the area
- Must be governed by an active Board of Directors
- Must provide services on a non-discriminatory basis with written policy attesting thereto
- Must provide a prospective budget specifically related to Health System's service area
- Must advance the mission of Wayne Memorial Hospital and WMHF Community Grant Program
- New organizations may apply for start-up funding in their first year of operations only; must have approved 501(c)(3) status or have applied and be awaiting approval
Eligibility Requirements
- No extensions of application deadlines allowed for any reason
- Funded programs must provide Progress Reports in format and frequency determined by WMHF
- Programs will not be considered for re-funding in subsequent years without WMHF-accepted Progress Reports and new fully-completed applications
Application Requirements
- WMHF announces acceptance of applications in May each year for the new grant period
- WMHF Community Health Committee reviews all applications and serves as allocations committee
- Committee members may include WMHF Board members, other Health System Board members, or community representatives not affiliated with Wayne Memorial Health System
- Committee members must disclose conflicts of interest and abstain from voting on applications where conflict exists

Application Process
- In May each year, WMHF announces acceptance of applications for the new grant period
- Applications for Support, Application Guidelines, and deadlines are made available by WMHF
- Submit fully-completed application by announced deadline
- No extensions of deadlines allowed for any reason
- Include prospective budget for the service/project/program
- Provide evidence of 501(c)(3) non-profit status (or documentation of application if newly forming)
- Include non-discriminatory services policy
- Applications are reviewed by WMHF Community Health Committee
- Approved applicants must provide Progress Reports on funded projects in format and frequency determined by WMHF
