Geographic Scope: Primary service area of Wayne Memorial Health System - Wayne and Pike Counties, PA, and the Carbondale and Forest City, PA areas

Vision Statement: A progressive, culturally and economically healthy community supported by an active, locally-controlled healthcare system dedicated to enabling the highest possible quality of life for all residents.

Mission Statement: To support community organizations whose endeavors are focused on injury and disease prevention and treatment and the promotion of good health for residents in the Wayne Memorial Health System service area.

Background: Wayne Memorial Hospital Mission (WMH CARES) is to provide quality healthcare with compassion, advocacy, respect, excellence and service, to promote wellness in the community and to be pro-active in responding to community needs.

Eligibility Requirements

  • Applicant organizations must be 501(c)3 non-profit corporations
  • Must provide health care services, health education and human service programs that improve health and well-being of community residents in the primary service area
  • Must demonstrate that the service/project/program is not an unnecessary duplication of services already provided by another organization in the area
  • Must be governed by an active Board of Directors
  • Must provide services on a non-discriminatory basis with documentation of policy
  • Must provide a prospective budget specifically related to the Health System's service area
  • New organizations may apply for start-up funding in their first year of operations only and must have approved 501(c)3 status or have applied and be awaiting approval
  • No deadline extensions allowed for any reason
  • Funded programs must provide Progress Reports (format and frequency determined by WMHF) to be considered for re-funding in subsequent years

Application Process Timeline

  • In May each year, WMHF announces acceptance of applications for the new grant period
  • Applications, guidelines, and deadlines are made available at that time
  • WMHF Community Health Committee reviews all applications and serves as allocations committee
  • Committee members may include WMHF Board members, other Health System Board members, or community representatives not affiliated with Wayne Memorial Health System
  • Committee members must declare conflicts of interest and abstain from voting on related applications

Application Process

1. Timeline: Applications are announced in May each year with specific deadlines provided
2. Required Materials:
- Fully-completed application form (available when announced in May)
- Prospective budget for the service/project/program
- Documentation of 501(c)3 non-profit status
- Copy of non-discriminatory service policy
- Evidence of active Board of Directors
3. Review Committee: WMHF Community Health Committee reviews applications
4. Reporting Requirement: Funded programs must submit Progress Reports in format and frequency determined by WMHF
5. Re-funding: To be considered for re-funding in subsequent years, organizations must provide WMHF-accepted Progress Reports and submit new fully-completed applications
6. Important Note: No extensions of application deadlines are allowed for any reason
