Urban Youth Ministry Certification Program

Application Process

1. Complete Certification Application Form including:
- Personal information (name, email, phone, address)
- Church/ministry/organization affiliation
- Educational background
- Demographic information
- Ministry experience and role details
- Number of years in ministry and youth served
- Current ministry vocational status (full-time, bi-vocational, volunteer)

2. Essay/Narrative Questions:
- Why do you want to enroll in the Certification Program?
- How will your current ministry, professional, and personal responsibilities support your involvement?
- Select level of support from ministry/profession/family

3. Financial Section:
- Indicate payment method: Pay-in-full ($1,645) or Payment Plan ($1,735)
- Pay $100 deposit (enrollment) or $50 deposit (alumni referral)
- Authorize monthly auto-payments if applicable
- Accept $35 fee for failed auto-payments

4. Referral Information:
- How you heard about the program
- If referred, name of person/organization
- Identify other group members from same organization (for group discount)
- Provide payment contact information if applicable

5. Agreement & Consent:
- Sign Certification Program Covenant Agreement
- Confirm understanding of attendance policy (>1 absence = dismissal)
- Agree to final enrollment only after payment received
- Consent to media release for photos, videos, and images in perpetuity
- Provide T-shirt size preference (Heather Sapphire color)
- Confirm accuracy of all information provided

6. Submission:
- Complete online form
- Type full name as electronic signature
- Submit for review
