In the U.S., 1 in 3 adults live with some form of cardiovascular disease. Those facing health disparities experience poorer health outcomes and shorter lives. That’s why health equity matters. That’s why the American Heart Association’s Social Impact Funds exist.

The Social Impact Funds support community-led transformation by strategically deploying capital to social entrepreneurs who are using their lived experience to tackle three vital social drivers of health.

We’re looking for organizations that are translating evidence-based science research and clinical know-how into solutions that address health care access and quality, food security, economic empowerment or a combination of these three social drivers of health.

Special Initiative: Loneliness & Isolation

Applications for projects and programs that address isolation and loneliness in older adults and support for caregivers are accepted year-round.
